MEMBERSHIP REGISTRATION DIRECTIONS
SECOND Semester Membership applications are now open!
Whether you’re an interested student or a returning member,
we are excited that you have chosen to consider joining CSF.
Listed below is all the necessary registration information.
Note that ALL RETURNING MEMBERS MUST REAPPLY.
If you're a first semester member, you do not need to reapply
during second semester though.
Applications consist of an online application form (which as a medical form on the last page), a digital version of your last semester transcript, a member agreement form, and a recommended member donation of $10.
ALL APPLICATIONS ARE DUE FRIDAY, JANUARY 20TH 2022 BY 11:59PM for online materials AND 3:25PM for in-person materials.
To be turned in digitally via email, send to email@example.com with the subject heading as "Grade Level_First Name_Last Name"(examples: 9_Mickey_Mouse or 12_Donald_Duck).
Online Application Form -- including the medical form on the last page -- (Fill out the PDF and screenshot! Previously some members had their info erased even after saving, so screenshot to make sure that doesn't happen)
A digital copy of your most recent semester grades [can use the unofficial transcript from Infinite Campus] - note: if you're a freshman, please obtain a transcript copy of your middle school grades
NOTE for 2nd semester applications: because 22-23 first semester grades are not finalized yet, please use grades from the last school year when entering courses
To be printed and turned in in-person to Mr. Kitchen's room (Room 111) - please put your NAME and GRADE on everything you turn in
Member Agreement Form (To be printed out and turned in to Kitchen in Room 111)
Recommended member donation of $10 (cash or check to Lynbrook ASB, paper clipped/attached)
