MEMBERSHIP APPLICATION FORMS

Membership applications open Friday, 09/13 and are due Friday, September 27, 2024!

Members MUST read the information on the "Membership Registration Directions" tab under Important Forms before proceeding with the application process.

On top of the application, you MUST turn in some form of proof for your grades. We ONLY need to see your grades from the past semester. However, you do have the option of submitting your full unofficial high school transcript if it is easier for you. Please email your completed application to lynbrookcsf2425@gmail.com with the subject line Grade Level_First Name_Last Name (e.g. 9_Mickey_Mouse) by 11:59 pm on Friday, September 27 2024. Please make sure to turn in the hard copy of your Member Agreement to Ms. Perez's cubicle in the office or one of the officers..
