MEMBERSHIP APPLICATION FORMS

Membership applications are open Friday, 01/19 and are due Friday, February 2, 2024!

Members MUST read the information on the "Membership Registration Directions" tab under Important Forms before proceeding with the application process.

On top of the application, you MUST turn in some form of proof for your grades. We ONLY need to see your grades from the past semester. However, you do have the option of submitting your full unofficial high school transcript if it is easier for you. Please email your completed application to lynbrookcsf2324@gmail.com with the subject line Grade Level_First Name_Last Name (e.g. 9_Mickey_Mouse) by 11:59 pm on Friday, February 2, 2024. Please make sure to turn in the hard copy of your Member Agreement to Mr. Kitchen in Room 111.
